Accessing the Categories/SubCategories from the WP Table

    I need to desgin a data entry form for people who are far from technical. They need to be able to select from a drop down box a category from the WP category list. Also the subcategories.
    So they need to able to select DREW STREET which is a subcategory of STREETS, 1ST WORLD WAR from the category WAR etc.
    I am a little confused from looking at the tables what is what?
    I have looked at the WP table structure diagram and read the info.

    Can anyone help me? Know of a solution?
    I need to be able to put in a drop down box on a form (CCFORM??) all the SUBCATEGORIES of a CATEGORY for a user to select.
