Account Creation Not Working

I have “Allow customers to place orders without an account” unchecked but when you go to checkout, there is no option to create an account. No username/password fields. I want all my customers to create an account when checking out, but there is no option to do so. Do I have to manually add these fields somehow? Also even though “Allow customers to place orders without an account” is unchecked, it still allows me to checkout without creating an account. Any help is appreciated.
