Account log-in and forgotten password problems

Hi

Multiple issues, probably all related:

1. User needs to log-in twice to /My-Account
On trying to log-in the first time the page just refreshes and then on entering log-in credentials the 2nd time it works.

2. Same issue for reset password

3. I tested forgotten password function from my own account (my WordPress admin account) and I received a forgotten password link via email. I registered a new account just as a customer and I do not receive a password reset email.
