Activating plugin causes media library to not load

This occurred with 1.4 and 1.4.1:
I noticed that since activating this plugin, clicking media->library loads a blank page. I’ve deactivated the plugin and the library comes back.This occurred with 1.4.1:
My file upload ( FORM ) isn’t working on Chrome Version 32.0.1700.107 m but does on firefox v26.0 & v27.0
