ActiveCampaign Integration

I switched from Mailchimp to Active Campaign for my email subscribers. I was using this plugin for my mailchimp subscribers and now I would like to use it for my active campaign subscribers. In the plug in, it requires code to be placed in the “form action” section. Where do I get that code in active campaign? Has anyone successfully set up active campaign with this plugin?
