Add customized field to form

Thanks for sharing your Sendy plugin.
I’ve made updates to our plugin to fit my needs and I’ll want to share them with you if you are interested.

• Add custom fields to widget configuration form (text button, placeholder)
• Bootstrap compliant HTML markup for the front form
