Add email recipient popup not working

I just installed this plug-in to experiment with it to see if it would work for the non-profit organization’s website I administer.
I am most interested in the email routing feature. Rather than publish 50 or 60 different email addresses on the site, I’d like to create a form that when the visitor chooses their county of residence, the notification email will be sent only to their county’s representative.
I created a dummy form, (in draft, not published) and click Email notifications in the left sidebar, then “Add email notification”. A popup box comes up with space for a subject line and message. But when I click Recipients, Advanced, or either of the other two tabs at the top, nothing comes up but a blank page.
