Add fields and filtering to comment form

I would like to add 2 custom fields to comment form, as this sample: https://www.lucebeauty.it/products/purifying-serum-30-ml

and then, filtering reviews using these fields

Fields are:

AGE (checkbox)
18-25
25-35
…

SKIN TYPE (select)
Dry Skin
Normal skin
…
