    What worked for me is changing line 1226 from:
    .enews #subbox {
    to
    .enews #subbox, .enews #subbox1, .enews #subbox2 {

    In fact, I am trying this thing on my localhost. But I can put it on one of my live site after sometimes.

    Now I am getting only Email Field.

    Can you confirm there is a field name filled in for first name and/or last name? If those fields are blank, they won’t be displayed.

    There is no field name for first name or last name in the plugin except Form Action, Email Field, Hidden Fields, Input Text and Button Text under Google/Feedburner ID.

    It sounds like you aren’t running the latest version of the plugin. Can you confirm you’re running the latest version 1.0.3 (you can find it Plugins->Installed Plugins)

    The name fields were added in 0.1.6.

    That was an issue. I’ve updated to 1.0.2. But not sure what to write on those fields?

    It depends on your mailing list service. MailChimp, for example, typically wants FNAME in that space. Aweber usually wants name in that space.

    On your mailing service’s website, get their code for creating a subscribe box that includes a field for name, use the <input type=”text” name=”FNAME”… to help tell you what to add there.
