add form HTTP ERROR 500

  • I used this plugin and like it before.

    But now I can’t add any new form in my new site.
    When I add a form and save , it show “HTTP ERROR 500”.
    I tried to disabled all plugins or change my themes, it also not working.

    And I tried to install WPQSF in other site, it working normally.

    Can you tell me where error is?

    The reason can be vary. Turn on your debug mode on to see any error message.
