Add images dont work

Hi, i would need more details to help you, can you paste here a link to image you are trying to upload and it does not work? is the problem always with the same images or is it random? and is there some error message when the file upload fails?

In other words i would need some instructions on how to replicate this error on your site? I tried uploading a couple of files to the gallery but it worked fine for me so far.
