Add more fields on form ?

I also have the same exact question. I cannot figure it out.

I have same situation here,

i want to implement an user input (Dynamic input to form) Click here to send email to : (when the radio option clicked) the jquery handle a add Input, same as this https://sroucheray.org/blog/jquery-dynamic-form/ but i dont know how to implement it.

You must go to smcf.php and look for function footer(), and there you can edit easily the HTML.
