Add posts' category above title, and date below title

Hi!

I’m working on a child theme of Twenty Twelve (offline with MAMP). On the main blog page, I would like to show the Category of each post above the title. I would like for the category name to be ‘clickable,’ like it is when shown below the post.

I also need the date to appear below the title of the post. This doesn’t need to be clickable (but fine if it is).
