Thanks. But it seems all form element as well instruction field can only place on separate line. Is there any way to place them side by side?

Every form item except for the Instructions field can be arranged in multiple columns by using the Field Output option.

Great. But I still have two problems:
i) In the select item, it seems I must select one of the option by default. Is there any way to make a selection box with default like [—-Please select—-] and user must select something if it is a required field?
ii) I have two fields, A and B. A is a radio button. B is a text box. B is required only if A is selected with certain value. Is there any way to do this?
