Adding additional fields to Checkout form

  • I have set up Book a Place Pro for a client and they now wish to get more information from people when the fill out the Checkout form. They would like to have two extra fields on the form and for both to be required.

    Edit: They would also like to have a tick-box on the form too.

    Can you advice what steps I should take in order to achieve that?
