Adding an email form to Blog…

After installing a WP blog and Semiologic, I’d like to add an email form including “First Name” and “Email Address” to add subscribers to aWeber or Autoresponder Plus.

I’d like to add this to a static home page or the sidebar.

Can anyone tell me step-by-step how to do this?
