Adding fields to calculator

I had an older version of the plugin on our site for several years. Recently we noticed that the totals were a little off. I have reinstalled the plugin. Now I no longer have the Property Tax field, Property Insurance field, the HOA field or the PMI field.

How can I add those fields back into the calculator, like the working example.

Also, how can I get the results to automatically show up with the amortization below the calculator?
