Adding image above icontact widget

I am looking to offer a free report in exchange for signing up to my newsletter. I have added the icontact widget to my sidebar….so far so good.

Now I would like to add an image above the icontact form to draw in the readers attention and offer them a free report.

I have added the image however it doesnt look at is should and I would appreciate some guidance as to how to fix my problem. Right now I have the image embedded within the contact form code….not sure if I should have it like this or there is a better alternative to this. Thanks!
