adding multiple guestbook fields

  • I’m trying to recreate a former pupils page for a school website. the original allowed ex pupils to enter details into a multi-field questionnaire, which, on approval by admin, would appear as a page with the questions and answers displayed eg.
    Name: Joe Bloggs
    Year you left: 1997
    What were your fondest memories: 3.30 pm every weekday
    etc.,
    etc.
    I’ve installed DMSGuestbook but that appears to have only 1 customisable additional field.
    Any plugins anyone knows of with this capability?
