Adding new form does not work

I had the same prob. but found the solution.

check your base url for mautic

in my case there was a trailing “/” after my base url
as in
https://mautic.website.com/

removing the trailing “/”, worked for me
https://mautic.website.com

This issue was related to the Base URL field, which was causing the problem in the case of the field having the trailing slash. The issue has been resolved.
