Additional Field to Form number 1

Hello,

First of all, I would like to thank you for this great plugin and I’m very much impressed of how it works as well on the form designs. I have been using the plugin now for almost a month I think and no errors ever encountered unlike other optin plugins I used…

There is just one thing I would like to ask, is it possible to add another field to form number 1? I mean I want to add a Phone filed in it, how is it possible?
