Admin Client Database

Hello community.

I’m imagining creating a way to create an admin library database for clients information that they submit into an online intake form on the website.

For example, when you go to the doctors office, and you fill out the paper work and give that to the receptionist, and the receptionist enters that info into a profile created, so every time you visit the doctor, your the answers to your questions and information are there in that client profile.

I would like to know if there is a plugin that allows me to do something like this?

User would fill out questions from the contact form element I have created, then when they submit that form, it is sent to another page I create, that would take that information and create a client library.

I’m not sure if that is the terminology.

I’m curious what plugin a medical website, or gym or massage center would use?

I already have woo commerce installed, yet this would be something more than an account where user can see their services purchased.

What I am aiming to create is a client library.
