Admin & workers are unable to receive a message

Hi There,
after filling a form only customer receiving a message however i have enable mail receving for both customer, Worker , admin will receive a message but i dont know why this is happeningand also while customer receive a msg mail address is showing wordpress how to get rid of wordpress text
