After form submission – cant load table

I have tried with both Contact Form 7 and Forminator but get the same issue.

I can integrate with the form ok and setup the columns ready to go. Everything look good with no row data and it displays on the front-end fine.

As soon as i submit a form, the Tablesome table will not load in both the front-end and the admin portal. (no errors, just blank page).

If i look in the backend mysql database, the form data has come through correctly, so looks like just an issue with displaying it.

Form has 3 text fields and an uploaded image.
