After latest update plugin broken

  • The plugin has fields to create a new form (there isn’t a field choice like there was in the previous version). I fill them out with (correct) data, hit the Create Form button and get blank page with “Choose a form”. So basically the plugin does nothing. I have deleted the plugin and reinstalled. I have it running on MAMP with PHP 7.3.7
