After moving wordpress page, forms are not showing

The form page looks fine, worked before I moved the site.

Strange thing is, it is showing on mobile, but not on desktop.

Thanks for the link.

Please note that with mobile it is not rendering too.

The issue might be connected with your theme, it throws some JS errors which make our form invisible, though it exists on that page.

I have made a screenshot for you to show the errors and the form that can appear if our JS code runs without any issues.

You can try to switch to another theme and check again.

If you still want to use the previous theme, then please contact the theme developers to fix the JS errors.
