After upgrade, contact information no longer displayed

As the subject suggests, after an automatic update to the plugin, the Address, Phone, Email portion is no longer visible.

I’ve ensured nothing has changed other than this and turning address information off and back on does not seem to do the trick.

Note: It wasn’t until after I disabled the email form (which I don’t recall enabling), THEN turning off the contact details <saving> THEN turning contact details back on did this actually fix.

I have confirmed several times that I have to turn off both the Email form AND the Contact Details, save it, and THEN turn contact details back on and save before the contact details will come back.

I guess consider this a bug?
