Ajax Contact Form, Not working?!? Please Read!

I’ve been trying for weeks to get a successful contact form up!
No progress YET! >:o
I’m looking to use this form :

I’ve done the following for now:
-Downloaded the source files.
-Created a contact template
-Inserted the script codes
but the form still doesn’t work!!!
