Ajax in custom page template

I have a custom page template. This custom page have a form with 3 dropdowns and a submit button. I need to use ajax (in the same page. i have only 1 custom page template) to prevent the loading of the page when the user submit it.

But I don’t know how to use this properly. Kindly help me.
