AJAX Survey-like Slider-ish Call to Action Idea

This idea is essentially a conditional call-to-action “form”. I can’t find anything like it, but I may be looking with the wrong keywords. Any ideas or direction is appreciated. Here’s basically how this would work (for my church’s website)…

– I’m on the fence when it comes to belief in God
(user clicks this option and is therefore given these > options which then leads to the ~ option, etc.)
> I’m open to meeting up for a drink or coffee with a Midtown leader
~ Input name, email, phone, part of town (east, central, west, etc.)
> I want more info about your mid-week meetups
~ Input name, email, phone, address
> I’ll just keep browsing your site
