Akismet Settings page blank

(it appears I had hijacked another thread, so I’m starting this one for my issues. my apologies)

I am running WordPress 4.2.2, Akismet 3.1.1. My Akismet Settings page is blank other than the word Akismet at the top.

I have disabled all other plugins and reset to the 2015 theme in an attempt to resolve, but the result is the same, blank page. Reinstallation of the plugin doesn’t resolve the issue.

I enabled WordPress debugging and received the following two lines above the word Aksimet:
