All text input prefilled with name value

I added multiple text input in application form fields template and when it displayed, its prefilled with value from Name text input. This becomes a problem for logged in users because suddenly all text input forms already filled with their name and they have to change one by one.

But, when i changed the label from Name to Full Name, all text input is not prefilled. I think there is something wrong with the logic of the code that makes other text input value shares the Name text input value.
