All thumbnails are broken

I’ve used this plugin several times on client sites before and it’s been great, but for whatever reason, it won’t work properly with my blog.
I have it all set up correctly, but the thumbnails are all coming up as broken images. I can see from inspecting element that it’s trying to pull weird file names for the images.
