All User topics/questions/comments need approval first?

Not all, only the first 3 (it’s sent in the settings) of new registered users. So the users who have more than 3 approved posts, their topics and posts become approved automatically. You can decrease the 3 to 0 in Dashboard > Forums > Tools > Antispam Tab.
