Alt and Title tags blank on Youtube Embed.

We are using your youtube video instructions found on your page here on the Description tab. First off its not the same as your Installation instructions word doc. Here you’ve got brackets [ around the tags ] On the instructions document you use < > around the tags. So that was confusing.
But my question is this. On this page: https://www.hairlosstalk.com/treatments/toppik-couvre/ You can view source and you will see that the ALT tags and the TITLE tags are empty. But they are not empty when I use your instructions to create the embedded code. An example (first video right sidebar):

Neither alt nor title show. View source shows:
Title tag is blank, and there is no alt represented at all. Please advise.
