alt text and image size from custom image field

Hi, I created a custom image field as per the documentation using get_post_meta which returns the image url. However I need to get the alt text and assign a custom image size if possible. Now I know that the only thing stored in the database is the image url, but the images are in the media library so I assume there must be some way of getting the data?
