Anyone have any idea how this build form was made?

    Not trying to steal it just really like the look at just can’t seem to figure out how it was made. I can see contact form 7 imprints but not sure if that’s the very bottom contact form.

    Anyone have any suggestions for a plugin that could be used to build packages like that?

    I’ve got Ninja Forms but god that would be a lot of fine tuning.
