Application Field Types All Change to “Section Heading”!

After spending a great deal of time working on the Application Form, updating after every two or three fields, suddenly every field type is changed to “Section Heading”. It’s happened twice now. The first time I was willing to believe it was a one-time glitch. Now I want to know why this is happening and how can it be stopped.
