Apply form errors

Dear @blueglassinteractive, I’ve created a test position and a form with 3 required fields which are name, email and a checkbox accepting privacy disclaimer
<a href="/en/disclaim/">I agree legal disclaimer</a>.Legal disclaimer text is showing twice, first as regular text and the second as it is supposed to be shown. In adition a Phone field that I havent added is shown.

Ok sorry @blueglassinteractive i thought it was fixed but it is not, i cleared the cache but also turned off the plugin, anytime that i turn it on phone field comes back again it doesnt matter how many times i try clearing all caches, i even tried wiping the whole site (as is a test site), new url is https://telnet-ri.prepro2.telnet-ri.es/job/test/
