attachment-sitemap.xml included in sitemap.xml but 404

    Can you confirm under admin dashboard > SEO (sidebar) > Search Appearance > Media Tab > that the ‘redirect attachment URLs to attachment itself’ option is set to NO and that the ‘show media in search results?’ option is set to YES? If so, then it should be expected that the attachment sitemap should show up in the sitemap index.
