Attachments Not Working, Black Arrow Spinning

I have the upload file in the attachments in my mail.

I have read through the forum, I have changed themes to see if it was a conflict, I have turned off plugins and done the plugin check. The crazy part a regular contact form without attachments works fine, but this one that I am trying will NOT work. I am at wits end, so I am asking for someone to please help me. What happens: The form shows up fine, however when you go to submit the black arrow just spins and no email is sent. Please don’t send me the link, I have been over and over it. Please help me to fix the problem.
