Audit log and last logins not complete

The Successful and Failed Logins lists are not showing logins for most admins. When I log in, I show up in the Logged-In users tab, but not in the Successful Logins (all) list. I can see activity in the audit logs from multiple users, but only one user is appearing in Logins lists. What could be causing this and how can I address?

The audit log is not capturing changes that occur within our theme’s configuration options. I’m not sure what to expect regarding what should and shouldn’t appear in the audit log – If settings and options are changed within themes and plugins, should those changes be tracked in the audit log?
