Baby growth chard

Hi,

I would like to a baby growth chart on my site (these are curbs based on weight and age of the baby). I can probably make them but I would like users to enter the age and the weight of their baby and have a dot appear on the graph.

Do you know any plugin that will allow me to do that? Or any other simple way to make it. I am very far away from coding.
