Backend Not Showing All Data Columns (Screen Options Not Saving)

Thank you for this plugin! It is very useful to us.

While the downloaded CSV file shows all the form fields, we’ve noticed that not all the form fields are showing in the backend itself.

I tried to change the Screen Options. The changes reflect while the Screen Options window is active, but once I click “Apply” to save my changes, the fields no longer show (see screenshot of Screen Options below).

I’m thinking this is either a glitch or perhaps I missed some kind of setting?
