Best ever for so many forms

I needed a survey form and I tried many plug-ins to no avail. Each had some “gotcha” issue. I stumbled across CaptainForm and tried the free version and it did exactly what I needed. It was simple, but very powerful and good looking. Since then I have gladly bought the premium version across several sites. It now handles all of my forms, from contacts, to surveys, to feedback, etc. I had two support requests and they answered up within 24 hours. Excellent forms plugin and excellent support in my experience with them so far. Plus they give away a ton of functionality even in the free version for those folks.
