Best form plugin for repeating use.

I need a simple form where members of a charity club provide their name and book one of two available meals. The data output needs to be readily accessible to authorised ‘non-coding people’ and the form content (meal choice) needs to be readily edited for (weekly) repeated use. (Online payment isn’t required.) Is there a free plugin ‘form builder’ that does this – either Classic or Gutenberg?
