Best Plugin for services? (inc Paypal)

Hi all,

I’m currently working on a site offering a service but can’t find the best plugin for this. Hopefully one of you guys have a great recommendation, ideally I’d like one in which the user inputs their name, email, selects a date, then selects the package they want and add it to a cart. And this cart links with paypal.

Here’s an image which best shows what I’d like, but I can’t find something for WordPress. I’ve looked at Woo Commerce but it doesn’t seem to fit my need.
