Black bars above and below video thumbnail?

We have a site where black bars are appearing above and below the video thumbnail images. We’ve attempted several times to remove them, but they continue to return. It seems that simply refreshing the page a few times will cause them to disappear (or return). Is this a bug seen by others? Suggested fix?
