blank page

Hi,
I have been using your plugin for a long time with no issues but i havent used it for a while, made a new page which i have to print but “a prediction form” which used to work, today is shown as a blank page ??

I had this issue before where you helped me but now i cant see the same fields in my admin section under your plugin to change something to resolved it as last time…

I have this link to my page, per haps you have time to look at this one day ?

i dont know if you can see anything from a not reg user ?
