Blank Screen After Submitting Form

    Our WP Donate form has stopped working. It has worked well until now. Now we are getting a white screen after clicking submit and the payments are not going through.

    The last payment we received through the WP Donate form was on 2017-01-05 at 15:17:52.

    Thank you for any assistance you can provide.
